At 5:39 AM PDT on April 3, 2019, one of our request handling systems unexpectedly failed. Normally, a failed system is automatically detected and removed from the pipeline, leaving requests to be processed by the healthy, redundant systems. Unfortunately, the automatic detection failed, resulting in unresponsive behaviour of the Thoughtexchange software applications for 1 hour and 8 minutes. Thoughtexchange's Development team was alerted via monitoring software and took action to correct the issue quickly. The root cause of the problem has been identified and we are actively making changes to ensure it doesn't re-occur.
